After years of improvement, crash data takes a deadly turn

The National Highway Traffic Safety Administration regularly collects and analyzes crash data, including the number of fatalities, severity of injuries and cause of the accident. For many years now, the agency has released even preliminary data with some fanfare, because the number of fatal crashes has been on a steady decline. So much so, in fact, that 2014 showed the lowest number of fatalities ever reported.

Technology doesn't eliminate pilot error

According to an article in The Wall Street Journal, "controlled flight into terrain" accidents accounted for 25% of all fatal commercial air accidents ten years ago. In 2014, however, they accounted for only 2% of all accidents and 13% of airline fatalities worldwide. "Controlled flight into terrain" refers to an accident in which a normal aircraft collides with the ground, water, a mountain or other obstacle after the pilot becomes disoriented or makes a mistake. According to the article, these accidents have become much rarer thanks to improved technology that warns pilots of an impending collision. 

TOO LITTLE TOO LATE: DELAYED DIAGNOSIS IS MEDICAL MALPRACTICE

The term "medical malpractice" can summon images of dramatic mistakes: surgeons amputating the wrong limb or leaving instruments in a patient's body cavity; improper use of forceps during childbirth leading to brain damage; medication errors that result in death. But medical malpractice can also involve doctors arriving at the correct diagnosis so late that the patient suffers harm.
